Case management is a supportive service designed to help individuals and families navigate the often complex systems involved in mental health care and daily living. Rather than focusing only on therapy sessions, case management takes a broader, practical approach—helping coordinate services, connect clients with community resources, and ensure that important needs such as housing, medical care, school supports, or financial assistance are addressed. A case manager works collaboratively with clients to identify goals, reduce barriers, and create a more stable, manageable path forward. This service is especially helpful during times of transition, high stress, or when multiple providers or systems are involved, offering consistent guidance, advocacy, and follow-through along the way.
